    Aesculus hippocastanum (horse chestnut)

    Regulates the peripheral circulation, the lymph flow, capillary action.

    In the case of Aesculus-Heel, therapeutical possibilities result for the treatment of varicose veins, venous stasis, varicose eczema, haemorrhoids, cutis marmorata, chilblains, acroparesthesia, rachialgia with numbness of the hands and lumbago (sacral pain as well as splinter pains, radiating from deep haemorrhoids). Aesculus- Injeel and forte have the same indications. In circulatory disorders, the following mixture has proved very effective: Aesculus-Heel, Arnica-Heel and Arteria-Heel 10-15 drops 3 times daily. Cruroheel S as alternating remedy for varicose veins and venous stasis. Aesculus-Heel acts on the venous vascular system, at the same time influencing the sensitivity (roots of spinal nerve), and thereby acting on haemorrhoids, varicose veins, venous stasis and rachialgia; also in the case of peripheral circulatory disorders, improves the cerebral circulation. The dosage is adjusted according to the disease, the symptoms and the stage of the illness: 10 drops 3-4 times daily; in acute disorders or painful conditions (alternating with Colocynthis-Homaccord) massive initial-dose therapy: 10 drops every 15 minutes until improvement occurs.

    The action of Aesculus compositum can be reinforced by injections of Circulo-Injeel and Placenta compositum; by way of a general stimulation of the detoxicating processes also through Hepar compositum (hepatic detoxication) and Populus compositum SR (oral preparation to stimulate the renal function), likewise through Lymphomyosot and Apis-Homaccord (both orally and as injections), which also exert a purifying action on the connective tissue and stimulate the renal function, further through constitutionally effective biotherapeutic agents (Psorinoheel, Galium-Heel, Schwef-Heel, Sulfur-Heel, Nux vomica-Homaccord, Calcoheel, etc.). For hypertension Rauwolfia compositum is indicated in addition, likewise, by means of Coenzyme compositum, Ubichinon compositum and possibly Glyoxal compositum, blocked enzyme functions should be reactivated. In the treatment of such circulatory disorders, therefore, the therapeutical holistic principle should be observed, which is achieved by the administration of Cor compositum, Cralonin, Aurumheel N drops, Melilotus- Homaccord, Glonoin-Homaccord and Cardiacum-Heel, in myocardial infarction with Strophanthus compositum, in apoplectic conditions with Carbo compositum. With regard to the hormonal control of the circulatory mechanism, one may also consider possibly Thyreoidea compositum (powerful action on the connective tissue, antineoplasmatic action) and Discus compositum (neural irritation with paresthesia in osteochondrosis, etc.).     Because of the individual constituents of Aconitum-Homaccord, the medication can be offered for the following conditions: acute infections, including influenzalike infections, rigors, acute rhinitis, otitis media, pharyngitis, laryngitis, bronchitis and bronchiolitis, pneumonia, and pleuritis. The medication can also be administered for acute and sudden onset of high fever with an undulating pattern. Finally, this medication can be given for coryza, catarrh, cough, sore throat conditions, nausea, and vomiting.

    Virusin 19 Forte issues from the reformulation of the former Virusin, with the purpose of strengthening the formulation:

    Virusin 19-Forte is a food supplement formulated based on plants such as astragalus, willow, acerola and turmeric, which are traditionally used vegetable ingredients; mushrooms such as reishi, cordyceps and lion's mane, which are traditionally used in Chinese medicine because of their multiple properties, and go beyond the stimulation of the immune system and provide a comprehensive action at the organism level. In addition, it is reinforced with a high amount of vitamin C, vitamin D, and zinc, micronutrients that play an important role in the proper functioning of the immune system; and astaxanthin, an antioxidant that enhances the beneficial properties of

    ASTRAGALUS (Astragalus membranaceus)
    Astragalus is an adaptogenic plant, from the Fabaceae family, which has been used in traditional Chinese medicine for centuries.

    Adaptogens feature because they increase their adaptability in case of stress and limit the damage that might occur. That is, in stress situations (for example, the entrance of a virus or bacteria into the organism) they improve the performance of different physiological functions of the organism.

    The main components of astragalus roots are saponins, polysaccharides and flavonoids.


    Saponins:


    Saponins are the most abundant compounds in watery or alcoholic extracts of astragalus, along with polysaccharides.

    Astragaloside IV, the most prevailing saponin in astragalus, it also contributes to the following functions:

    Immunomodulatory, increasing the immune response. In different studies, an increase in important elements in the response mechanism of the immune system has been confirmed. For example:

    • It stimulates the proliferation of T and B lymphocytes.
    • It stimulates the expression of the major histocompatibility complex and cooperates in the development of antigen presenting cells.
    • It increases the levels of Interferon-γ.

    Cardiac. This is favoured by the balancing effect of calcium, which can prevent damages to the heart muscle.

    Hepatoprotective. Because of its double function: on the one hand, the immunoregulatory effect protects against the liver damage induced by the immune system; and on the other, the antiviral effect provides protection against, among other things, the hepatitis B virus.


    Collagen synthesis. It features a dual action in the regulation and catabolism of collagen. It protects from the damage caused by UV rays in fibroblasts (FB). It promotes FB proliferation and prevents collagen degradation. However, astragaloside IV may slow down the development of fibrosis in organs such as the heart, liver, lungs, and kidneys.


    Antioxidant. It has inhibitory action on lipid peroxidation and protein oxidation.

    Polysaccharides:
    Its main function is the balancing of the immune system. Astragaloglucans are the best known.


    The research on the effects of astragalus on the immune system has been mostly carried out in vitro and in animal studies. The results of these studies show that astragalus stimulates the immune response:

    • Polysaccharides increase the number of T cells and their activity, and balance the expression of cytokines.
    • They also increase the activity of natural killer (NK) cells and activate macrophages.
    • Through their effect on the immune system, polysaccharides could also have
      antiproliferative effects on tumour cells.

    Likewise, polysaccharides feature an antioxidant, antidiabetic, liver-protective, anti-inflammatory and neuroprotective effect.


    Flavonoids:
    Flavonoids are pigments that are naturally found in plants. Isoflavones are the most
    representative in astragalus. Flavonoids belong to the category of polyphenols. Most flavonoids act as antioxidants that catch free radicals to render them harmless and thus prevent cellular damage.

    TURMERIC (Curcuma longa)
    In Virusin-19 Forte, the properties of turmeric are added by means of Novasol®
    Curcumicin, a 6% micellar extract of curcuminoids
    . Novasol uses turmeric in a fully
    water-soluble and pH-stable form, thus promoting an optimal oral bioavailability.
    In recent years, there has been a great interest in curcumin as an ingredient that provides
    multiple properties without any side effects.
    Properties to remark:

    • Anti-inflammatory activity. By acting as a scavenger for pro-inflammatory substances, such as nitric oxide and COX-2.
    • Antimicrobial activity. Evidenced, above all, at digestive level, with bacteria such as Escherichia coli or salmonella typhi. There are also references to inhibition with Staphylococcus aureus.
    • Digestive benefit. Research suggests that it increases the solubility of bile, helping in the elimination of gallstones and preventing them.
    • Skin health. Studies with curcumin have been proven to reduce wound healing time, improve collagen deposition, and increase vascular and fibroblast density in wounds, thereby improving healing. It is also associated with a beneficial effect as a proangiogenic agent, by inducing transforming growth factor beta.
    • Neuroprotective. It is associated with positive effect in diseases such as Alzheimer's (experimental studies in mice), thanks to the fact that it protects the brain from lipid peroxidation and nitric oxide-based radicals.
    • Cardioprotective. It is also related to the ability to regulate LDL cholesterol and TG, helping to prevent cardiovascular diseases.

    WILLOW (Salix alba)
    Willow is a tree of the Salicylaceae family. The bark is used, in which the most relevant
    assets of its composition are found: salicylic derivatives (1.5-11%).
    It features because of its analgesic and anti-inflammatory properties.

    Willow extracts have been observed to inhibit the production of proinflammatory products,
    reducing the expression of TNF-α and COX-2, as well as inhibiting the production of NO
    and the proinflammatory transcription factor NF-kappa B.

    ACEROLA ( Malpighia punicifolia)
    The fruit of the acerola or Barbados cherry is extracted from a tree that grows in the
    Antilles and north of South America.
    It has a high content of vitamin C and is used as a stimulant and modulator of the
    immune system
    . It has a high antibacterial activity and has been used as a dietary
    supplement and in the prevention of flu and colds.

    CORDYCEPS (Cordyceps sinensis)
    This ascomycete fungus grows at an altitude of 3,800 meters in the grasslands of the Himalayas. It parasitises the larvae of an insect (Hepialis armoricanus), and is known as “caterpillar fungus”. It is highly valued in traditional Chinese medicine, as a popular remedy for its adaptogenic properties: it improves health, increases physical and mental capacity, and increases energy.

    The sclerotium (a compact part of the mycelium) is used, together with the parasitised larvae.


    The most relevant active substances are: cordyceptic acid, glutamic acid and cordycepin.

    • Among its properties, the energising is the most outstanding. It features anti-fatigue effect, increases the use of oxygen and the elimination of lactic acid; other actions have also been
      seen:
    • It stimulates the immune system, because of its anti-inflammatory nature reducing
      proteins that increase inflammation, as confirmed in a Korean study published in 2014.
    • It protects the heart, by increasing adenosine levels, and reduces LDL cholesterol and
      triglyceride levels.
    • It balances blood glucose. It imitates the action of insulin.
    • Hepatoprotective. An investigation of the World Journal of Gastroenterology concluded that the fungus Cordyceps sinensis was able to slow down tissue fibrosis, reducing inflammation and improving hepatocellular function, thus delaying the development of cirrhosis.
    • Antidepressant, by increasing dopamine and adrenaline levels.

    REISHI (Ganoderma lucidum)
    This parasitic fungus that grows on trees and is widespread on practically all continents, has
    been used in traditional Chinese medicine for more than 2,000 years. In China it is called
    "Lingzhi" and represents a combination of spiritual potency and immortality. This adaptogenic
    mushroom symbolises success, well-being, divine power, and longevity.


    The carpophore is used, which contains a wide variety of bioactive compounds: terpenes,
    sterols, phenols, nucleotides and their derivatives, glycoproteins and polysaccharides. Its
    proteins contain all the essential amino acids and are particularly rich in lysine. The low lipid
    fraction of this fungus is rich in polyunsaturated fatty acids, which contributes to its healthy effects. Polysaccharides (β-glucans), peptidoglycans and triterpenes (ganorenic acids)
    are the most important active ingredients.
    Among its properties, outstand:

    • Immune system stimulant. It increases the production and maturation of T and B
      lymphocytes, the activation of "natural killer" cells, and an increase in cytokines (TNF-β, IL-γ).
    • Relaxing of the Central Nervous System. It counteracts the states of stress, anxiety
      and insomnia.
    • Cardioprotective. It inhibits the synthesis of cholesterol and reduces arterial
      hypertension.
    • Hypoglycaemic. An increase in insulin levels attributable to Ganoderane B has been
      observed.
    • Hepatoprotective. It helps liver cells regenerate and the metabolism of substances
      speed up.

    M LION'S MANE (Hericium erinaceus)
    Edible fungus of traditional use in Chinese and Japanese medicine used for gastric problems
    and loss of memory and lucidity. With adaptogenic properties.


    Mycelia and carpophores are used.


    It has components with important physiological activity; the most important and also the most abundant are the polysaccharides, among which the β-glucans stand out, and the terpene compounds (hericenones and erinacins).
    Its main functions are:

    • It strengthens the immune system.
    • Neuroprotective. It regenerates the tissues that favour neuronal connections. It improves
      the cognitive functions. It stimulates the nerve growth factor or NGF, hence its interest in
      degenerative diseases of the Nervous System (Alzheimer's and Parkinson's).
    • Cardioprotective. It can be attributed the property of lipid regulator, since it inhibits
      HMG-CoA reductase and protects LDL-cholesterol from oxidation.

    Contraindications / Side effects
    Patients treated with anticoagulants or with
    recognized vessel fragility should be carefully monitored during the therapy.
    There is no history of hypersensitivity to MD-HIP. It contains animal-derived collagen from porcine source. Patients with known hypersensitivity to any ingredient or excipient should be tested before use, making a spot injection into one arm and be monitored for 1 hour.


    Warnings and precautions
    Hip pain requires differential diagnosis for primary or metastatic cancer pain, referred nerve pain of lumbar origin, inguinal hernia.
    A slight reddening at the injection site may be due to a mechanical effect of the needle or to a skin reaction. The application might cause symptoms of burning/pain at the injection site, that usually solve spontaneously within 5-10 minutes after the treatment.
    Skin cleansing/disinfection is required before and after application. Pyogenic bacteria may produce injection site abscesses.

    Instructions in case of damage to the sterile vials

    Do not use if the seal is broken or if the vial is not perfectly sealed. Once open, the content of the vial must be immediately injected. Do not use if the package is damaged.

    Hip – Intraarticular injection

    The trochanteric bursa is located over the lateral prominence of the greater trochanter of the femur.
    There can be an anterior or a lateral approach to the hip.
    The patient is lying supine. The anatomic landmark is located 2 cm below the superior border of the greater trochanter in between its anterior and posterior borders. The skin overlying this location is
    marked and prepared in a sterile way. A 22 gauge needle is injected parallel to the floor and perpendicular to the femoral shaft.

    Note:
    Subcutaneous (s.c.) and intradermal injections: use a 12-16mm, 25-28G needle.
    Intramuscular (i.m.) injections: use a 40mm, 21G needle
    Intra-articular injections: use a dedicated needle.
    • When making subcutaneous injections, insert the needle at approximately a 45 degree angle
    • When making intramuscular injections, insert the needle at approximately a 90 degree angle

    STUDY

    INTRA-ARTICULAR ADMINISTRATION OF MD-HIP IN 7 PATIENTS AFFECTED BY HIP OSTEOARTHRITIS UNRESPONSIVE TO VISCOSUPPLEMENTATION. SIX- MONTH MULTICENTER TRIAL

    Experimental sites: UOS (Simple Operating Unit) of Rheumatology – San Pietro Fatebenefratelli Hospital, Rome.


    Pathologies considered: osteoarthritis X-Ray I-III stage according to Kellgren-Lawrence affecting the hip joint unresponsive to viscosupplementation with hyaluronic acid (6 patients) or hylan (1 patient) (2 ultrasound guided injections at least).


    Outcomes:
    1)assessment of efficacy using VAS scale and Lequesne algofunctional Index;
    2)NSAIDs consumption before treatment and during follow-up. 3)safety profile of MD-Hip.


    Patients enrolled: 7

    Treatment: MD-Hip (2 ampoules = 4 ml), 1 ultrasound guided intra-articular injection.

    Results
    1) VAS of osteoarthritis pain = from 6.15 (before treatment) to 4.23 (after 3 months), to 4.23 (after 6 months).
    2) Lequesne Index = from 1.94 (before treatment) to 5.9 (after 3 months), to 5.83 (after 6 months).
    3) NSAIDs consumption = from 7.57 (before treatment) to 4.25 (after 3 months), to 5.78 (after 6 months).


    – Author’s conclusions:
    1) MD-Hip showed to be effective (all the average values of the results at 3 and at 6 months after the last treatment have been statistically significant) and safe in patients affected by hip osteoarthritis unresponsive to viscosupplementation.
    2) The data suggest that the results can be evident from the very first injection and are stable for 6 months.
    3) The preliminary data offer new research opportunities in the field of intra-articular therapy.

  • Dosage of Heel Medications in Veterinary Medicine

    In the veterinary profession it is important to know the forms of presentation and specifications of applying and storing medications, as ignorance of all or part of this may be the cause of treatment failure.

    The different pharmaceutical forms set out by Heel in this guidebook are meant to convey general information relating to the products or ingredients taking into account the needs of the clinical veterinarian, for both small and large animals.     Subacute or chronic conditions: the dose is given less often (once a day to once a week), but for a longer period (4–6 weeks).

    Oral products (drops and tablets) have been developed on the premise that the animal’s owner carries on and completes the prescribed course of treatment at home. To this end the owner has to be told that once the products have been opened they should be used up within a limited time. The use-by date should in any case be observed.

    In any case of doubt about the right therapy with these oral products, e.g. in cases of a missing therapy result or reoccurrence of the previous pathology, the veterinarian should be consulted so as to achieve the best therapy possible.

    The ointment compounds, of which there are only two (Traumeel® and Zeel®), are aimed at reinforcing the oral and injectable forms of presentation, in disorders of the musculoskeletal system, since their sphere of action is localised.

    Finally, we refer to the injection solutions, which are the main instruments in the veterinarian’s toolbox, as they go a long way in extending the practicability of homeopathy in veterinary medicine.

    No specific storage conditions are required for the Heel medications; they should however be kept in a dry place with normal temperatures (room temperature or below) and not be exposed to direct sunlight.

    Dosage

    Tablets and drops

    They have no intrinsic taste and it is possible to mix them with a little water without them losing their effectiveness.

    It is preferable to administer them between meals. The general dosage is three times daily. In both cases one may increase the frequency of application as shock therapy in acute disorders.

    In the cases of those animals where it is difficult to proceed in this way, there are alternatives. Triturated tablets mixed with a little honey can be spread on the part of the animal where it tends to lick itself, for instance on a cat’s front extremities. The same procedure applies to drops. Another possibility is to inject or insert drops or tablets into a small amount of food, with horses into a piece of dry bread, carrot or apple and with cats or dogs, into a piece of cheese or meat.

    Ointment

    Apply multiple times daily, without strong massaging, to the affected area. If necessary an occlusive dressing may be used.

    Ampoules

    Heel ampoules may be used parenterally (subcutaneous, intramuscular or intravenous injection, …), taken orally and injected to acupuncture points, either individually or combined with other Heel injection solutions. The normal dosage is 2 or 3 times a week, although in severe cases it is daily, sometimes several times a day, until there is improvement.

    The advantages of an oral application of the ampoules, as there is no need for asepsis, are convenience and lack of complications (the customer himself can administer them), etc. With cats, the oral administration of an ampoule is often the only way for an oral treatment, as these animals do not tolerate the taste of the drops as they are an alcoholic solution. Despite the fact that the appropriate precautions and measures have to be taken for injection, this way is preferable, at least at the start of treatment, since it ensures the dosage is correct, quick and sterile.

    The best known ways of injection are:

    Intravenous: Administered in a matter of seconds. It is the most suitable way for severe and very acute cases. Its main features are the immediate onset of action, but often also the need to repeat applications in highly acute courses.

    With infusion of saline solution it is possible to add a beneficial homeopathic medicine in the same liquid or cannula. The medicine may either be mixed with the saline solution (if one wishes to maintain the effect) or administered first, right after the venoclysis (immediate effect).

    Subcutaneous: Probably the least discomforting route of injection. Often preferred in all species incl. small animals. Also possible in painful areas and at the acupuncture points, often taking effect after a few minutes.

    Intramuscular: The onset is slower. Often preferred in farm animals.

    Intradermal: The suggested way when neural effects are required. Injected in nerve segments or over the affected area.

    Local and pericapsular infiltrations: Help to solve an important clinical problem: the treatment of continued pain associated with musculoskeletal lesions, requiring some substance with analgesic properties that may be used successively over long periods of time and has no undesirable side effects.

    Intralesional or perifocal: For example, on occasions before suturing one can empty the contents of the ampoule (injectable) into the surgical wound or make little papules in the edges of the incision.

    Progressive auto-sanguis therapy: Also known as the gradual autohemotherapy, an innovation contributed by Reckeweg to the autohematic method described by Haferkamp. Basically, one can define it as a biotherapy consisting of injecting the patient intramuscularly with his own blood combined with appropriate homeo­ pathic remedies (see specific literature). Generally practised in chronic diseases, dermatosis and autoallergic processes.

    Recommendations

    The lumen or perimeter of the injecting needle may be reduced, since the vehicle of the injectable homeopathic remedies is of low viscosity. This aspect associated with the character of the substance (not irritating) means that the injection is not painful.

    Antihomotoxic medication must not be mixed in the same syringe with conventional medicines. Antihomotoxic medicines should be administered first.
